Individual Registration Form and Payment - this is required for all skaters!
Team Entry Form - This form is required for teams including: Precision, Show, Survivor, Family, Dance and Pairs.
Rules - Relevant rules and details about the events. The rules are also detailed on the usopeninline.com from the menu links
Amateur Card Single Day Entry Request Form - Required for non-USARS or FIRS registered skaters to cover the cost of liability coverage. This form is used to request the actual form which will be either mailed or provided on site. Each form has a unique number registered to the skater.
Sponsorship Form - For organizations and individuals that want to become sponsors of the U.S. Open Inline competition and U.S. Inline Figure Skating.
Program Advertising Form - Submit advertising for the competition program.
Practice Rink Rental Form - Request rental time at the competition rink. This is for exclusive use of the rink in hour increments.
Vendor Application Form - Request vendor space at the rink. This is submitted directly to the rink
Volunteer Application Form - Volunteer at the competition. Also please ask at the registration desk.
